Experience

Social Work Operations/Adult services - Argyll and Bute Council

(2024-10)

Adult services and social care (Hospital and Community)

  • Case Management & Duty: Managing a caseload of adults with varying needs; rotating on the Duty Team to respond to urgent referrals and initial inquiries.
  • Statutory Assessments: Conducting holistic assessments to determine eligibility for services, focusing on co-designing care plans with individuals and their families.
  • Hospital Coordination: Acting as a bridge between the ward and the community; managing Delayed Discharge reports and coordinating multi-disciplinary meetings to ensure safe transitions.
  • Resource Navigation: Facilitating referrals to internal and external services, including Telecare/Technology, home care, and financial advocacy (e.g., Bute Advice), the Befrienders (for socialisation), and the Crossroads (for carers support). Other referrals include the day centre and respite care.
  • Adult Support & Protection (ASP): Experienced in the Duty to Inquire process under the ASP (Scotland) Act 2007. Conducted initial inquiries to identify risks of harm and collaborated with MDTs to establish safety plans.
  • Adults with Incapacity (AWI): Practised in line with the Adults with Incapacity (Scotland) Act 2000 principles. Participated in Case Conferences to assist in the processing of Guardianship, ensuring the individual's past and present wishes are central to the legal proceedings.
  • Risk Management: Being mindful of balancing the 'Dignity of Risk' with the Duty of Care, specifically managing the tensions between autonomy and protection as outlined in the SSSC Code of Conduct.
  • Trauma-Informed Practice: Developed a deep understanding of how to communicate effectively with individuals in crisis, prioritising safety, choice, empowerment and collaboration.
  • Human Rights Frameworks: Gained practical experience applying the Equality Act 2010 to ensure non-discriminatory practice and to uphold the rights of marginalised individuals.
  • Multidisciplinary Collaboration: Gained insight into the importance of 'Professional Curiosity' when working alongside health, housing, and legal professionals to build a comprehensive 'big picture' of an individual's life.
  • Reflective Practice: Learned to utilise emotional intelligence and resilience to maintain professional standards under the high-pressure environment (including remoteness) of statutory social work
