I am a Level 1 qualified Social Worker with experience supporting children with disabilities, complex needs and families requiring statutory intervention. I am seeking a role within a Children with Disabilities team where I can deliver high-quality assessments, direct work, multi-agency coordination, and strengths-based support.

Social Worker – Disabled Children and Young People's Service - London Borough of Greenwich - London

• Acted as single point of contact for family and professionals at the beginning of EHC needs assessment process ensuring the legal framework is adhered to.
• Managed a caseload accordingly to ensure that the Service is compliant with the timescales set out in the SEND Code of Practice.
• Communicate regularly and sensitively with the family/young person throughout the EHCP process to explain the process and ensure their involvement in assessment and planning.
• Conducted assessments of children with learning disabilities under the Children's Act

• Engaged in statutory work across Child in Need (Section 17), Child Protection (Section 47), Children in Our Care, and Care Act pathways, supporting disabled children and their families with holistic, legally-informed interventions
• Worked in practice across children's and adults' legislation by applying both the Children Act and Care Act when assessing needs, risks, and eligibility for young people approaching adulthood
• Supported young people with SEND aged 14–25 through Moving to Adulthood planning, contributing to developing pathways that prepare young people for independence, adult services, and long-term wellbeing
• Acted as duty worker covering queries relating to social care.
• Presented allocated cases in panel for support packages
• Conduct safeguarding enquiries about children at risk of abuse or neglect, including self-neglect.
• Advocate for people and practising in a strengths-based approaches that promotes positive risk-taking.

Social worker – No recourse to public funds - DePaul Homeless Charity

• Provided support to individuals with No Recourse to Public Funds experiencing homelessness and destitution
• Completed NRPF assessments and identified safeguarding concerns including exploitation, rough sleeping, mental health deterioration and domestic abuse.
• Held a caseload of up to 10 individuals, delivering trauma-informed, culturally sensitive support.
• Completed holistic needs and risk assessments with young people aged 16–25, showing vulnerabilities, strengths, and transition-related support needs
• Carried out Moving to adulthood assessments of young people.
