• Advocated for individuals with dementia and learning disabilities, who were unable to advocate for themselves.
• Attended Multi-agency meetings to discuss and review care plans for the Individuals i was working with.
• Worked alongside a wide range of Professionals i.e, Social Workers, Care Workers, Care Providers and Care Managers.
• Completed and uploaded Case Notes and Reviews for each individual.
• Ensured that the needs of each individual I was working with was met to a high standard - providing effective support and intervention.
