Care Community Support Manager - NHS East Cheshire Trust - Eastern Cheshire

I work within NHS East Cheshire Trust, leading and coordinating care community projects across a defined locality in Eastern Cheshire. I use population health intelligence to identify health inequalities, unmet need and opportunities for targeted neighbourhood interventions, working collaboratively with Primary Care Networks, local authorities, VCSE organisations and health partners. I work for Communities and Partnerships for Cheshire East Council (Local Authority). My role is improving the quality of life for various groups in the community. We involve ourselves for identifying gaps in service provisions; we work alongside several organisations including healthcare, police, voluntary/faith sector young people and education.
My main role is to implement and support range of activities and projects, within neighbourhood partnership plans/ priorities to increase health and wellbeing in the community, empowering residents in accessing services, community links and our connected community centres. I have previously worked for Pathways CIC, full- time, which is a third sector organisation supporting health and wellbeing initiatives for the population of Cheshire East. My job is to support and engage with different Black Ethnic Minority Group (BME) communities, to understand local health provision, access services and service pathways. The team and I recognise the importance of positive 'Mental Wellbeing' for individuals and improving their opportunities by way of understanding the issues to improve the quality of care.
My other current role as a Health and Employment Advisor entails dealing with clients with health and mental wellbeing issues. My aim is to support and guide my clients, encouraging them back into employment or in a suitable voluntary/ training position. This involves effective communication with each of my clients as I prepare, deliver tailored actions plans, identifying each personal circumstance to suit every client.

This is a part time job in Crewe Hall Brasserie Restaurant, which helps me interact with customers to improve my interpersonal skills. The position requires high organisational skills and face-to-face customer services. Taking orders and serving food and welcoming customers into the restaurant are also part of the profession.
I have previously worked in other restaurants, which has made me a much more confident individual, which I can now utilise this experience in delivering high level of consistency in the quality of all services.
Special Educational Needs Mentor - Monks Coppenhall Primary School

This was a temporary role at the local primary school. My main role was to mentor, assess and support children with special educational needs, Literacy and Numeracy. I have carried out one to one sessions with children aged between 5-11 year olds.
I had also volunteered in my spare time with Key stage 1, observed the learning techniques and seen how teachers arrange their planning and preparation for every subject. This gave me more insight and awareness how important organisational capabilities can be involved in providing the requirements of special needs.
