Care of elderly medicine. I worked alongside the stroke team to admit and assess stroke patients for appropriate acute management. Worked with the wider team consisting of stroke therapists, nurses, dieticians to carry out patient specific plans and transfers in weekly MDT meetings.

Carried out independent ward rounds for stroke inpatients, assessing their progress, creating plans, expediting transfer and using a compassionate approach to help build trust when discussing complex care and End of life discussions with patient families. As SHO in COE medicine my role included:

• Clinical Assessment and Management: I assessed elderly patients and developed personalised care plans. I managed chronic and acute illnesses in older adults.
• Team Collaboration: I worked with a multidisciplinary team to plan and coordinate patient care and discharge.
• Acute Care Provision: I provided urgent medical care, handled emergencies, and stabilised patients in acute geriatric units.
• Communication: I explained diagnoses, treatments, and prognosis to patients and families, offering support and reassurance. Managing Geriatric Syndromes: I diagnosed and treated conditions like delirium, dementia, and falls, and took preventive measures.
• Medication Management: I reviewed and adjusted medications to avoid complications and meet patient needs.
• Ethics and End-of-Life Care: I made ethical decisions and provided palliative care and support for end-of-life issues.
• Education and Training: I supervised medical students and junior staff and participated in professional development
• Attending/Presenting a given topic in the departmental academic sessions.
• Teaching and guiding the new doctors joining the department.
• Documentation: I maintained detailed patient records and documented assessments and treatments accurately.
• Research and Quality Improvement: I engaged in research and initiatives to improve geriatric care and patient outcomes.

Acute Medicine. Attained a high level of proficiency when conducting in-depth patient assessments, encompassing meticulous screening for acutely unwell patients and managing symptoms and complications, followed by triaging to appropriate medical specialties for follow up care. Worked alongside a wider AMU team, to collaborate and coordinate patient care and actively participate in ward rounds, medical meetings, and case discussions.

Escalating and asking for assistance in complicated admissions. COVID Medicine Delivery Unit (CMDU) In my role at the clinic, I provided medical care for high-risk COVID-19 positive patients experiencing complications. This involves conducting morning and afternoon ward rounds, documenting patient information, and prescribing appropriate antiviral medications based on their individual health needs and medication history.
