I am a licensed Social worker with a LBSW degree. I have worked in skilled Nursing facilities for almost 7 years straight having done MDS assessments, Social histories, social service quarterlies, discharges for those that are returning home as well as getting medical equipment and setting up home health services.
Social Services Director providing crisis interventions, assessments, scheduled and attended care plan meetings, assisted with NOMNCs when MDS case manager was not available, contacted home health or hospice when a patient discharged home, and reached out to home health and medical supply companies to make certain patients had safe discharges.
