Medical Social Worker for nearly a decade to now, with a focus in Home Health. Some of the corporations I worked for are Suncrest (now as of Dec 2nd Adoration), Baptist, Medi, Gulf Coast and Angel's Home Health. Some of the roles I do, but not limited to:
Perform initial evaluations and assessments.
Assesses & documents biopsychosocial: H&P, diagnosis, medication adherence, overall safety, understanding of disease process/management, community resources, transportation, cognitive, emotional, mental, pain, client’s family/friend support, spiritual support/needs, end of life planning, need for outside resources or facilities
Creates short- & long-term Care Plans to include referrals, orders
Educate clients and family on Medicare/Medicaid/VA, and non-profits
Implements proper level of care identification, such as crisis-management, suicide prevention resources, solution-focused therapy
Identifies needed durable medical equipment through proper educational linkage
Completed Internships at Epilepsy Foundation NWFL, and Masters level at Sacred Heart Hospital as a medical social worker in the hospital setting
Child Protective Investigator (2012) specialized in sexual abuse, trauma, death cases
Responded to hotline reports, determined immediate risk; notified proper authorities; dealt with sexual and physical abuse, and death cases
Conducted Child Safety Assessments; contacted family/victim(s) of abuse, investigated neglect and/or maltreatment allegations, searched medical records, priors, and background checks
Performed wellness checkups and collaterals; explained HIPAA rights; arranged for emergency placement for children at risk; conducted mediation and resolution of conflicts
Also have many classes completed in medical, such as anatomy 1 and 2, patho, microbiology and med terminology, well educated in this field
Masters in Social Work from University of West Florida (UWF)
Bachelors in S.W, and Minor in psych (UWF)
Have up to date BLS advanced CPR In LCSW internship, takes 2 yrs will complete before end of 2027
