Provide comprehensive primary care to patients across the lifespan, including preventive screenings, acute illness diagnosis and treatment, and chronic disease management. Conduct patient assessments, order and interpret diagnostic tests, and develop individualized treatment plans via telehealth or in person. Prescribe medications and manage ongoing pharmacologic therapy in accordance with state scope of practice.
Manage chronic conditions such as hypertension, diabetes, and hyperlipidemia through regular monitoring and care plan adjustments. Perform well-visit exams, BHRT, Aesthetic Services, Peptide Therapy, immunizations, and health screenings for patients of all ages. Educate patients and families on disease prevention, medication management, and lifestyle modifications.
Collaborate with physicians, specialists, and ancillary staff to coordinate continuity of care. Maintain accurate, timely documentation in the EHR in compliance with HIPAA and clinical standards.
