- Capture details of clients on ART for less than one year and newly initiated patients on ART on the CMM excel spreadsheet, and plot all due appointments.
- Capture clients with high viral loads, pediatric clients, preArt clients and pregnant women.
- Phone clients and conducts daily linkage to a CHW or any appropriate community person for those who missed their appointment; pay a visit to clients who missed their appointments, patients who are not virally suppressed, and patients needing additional care.
