• Conducted Best Possible Medication Histories (BPMHs) on admission under pharmacist supervision, including patient interviews and verification of medication information.
• Participated in interprofessional communication with pharmacists, doctors, local GPs and other healthcare professionals to clarify and verify medication information.
• Gained experience with discharge medication processes and patient counselling under pharmacist supervision.
• Participated in day to day ward based clinical pharmacy activities with a focus on safe, patient-centred medication management.
