Population Health Strategy
- Designed and implemented population health management (PHM) strategies and clinical programs to support organization’s mission and values, and for meeting member needs across the continuum of care.
- Created a robust registry of clinical programs and categorized them into risk tiers (keeping members healthy, managing members with emerging risk, patient outcomes across settings, managing multiple chronic illnesses) to ensure member needs are addressed and met under each risk stratum.
Clinical Disease Management Programs:
- Formed inter-departmental teams to develop condition-specific clinical disease management programs to address health needs of members with chronic conditions (asthma, diabetes, COPD, behavioral health needs).
- Partnered with Rhode Island Department of Health (RIDOH) to design and implement an evidence-based ‘Asthma Home Visiting Intervention Program’ led by community care coordinators with a goal to reduce preventable asthma emergency department visits and hospitalizations.
Health Equity Projects:
- Chaired organization’s health equity committee and created a forum to have open dialogues on initiatives to address health disparities by partnering with community organizations.
- Identified three focus areas - maternal health, childhood obesity, and behavioral health (opioids overdose); supervised sub-committees to design and present their health equity project proposals to the executive leadership team.

- Developed a robust project management plan for ‘Ambulatory Pharmacy Program’ targeted to patients with uncontrolled hypertension and diabetes by providing clinical pharmacy support and interventions.
- Conducted internal surveys and performed comprehensive analysis on dashboard functionalities, visuals, and reporting methods to enhance organization’s quality and clinical performance management tools.
- Co-designed and led ‘United Against Racism Asthma Program’ that aimed to reduce hospitalizations and acute visits of children with uncontrolled asthma conditions.

- Mapped out a drive-thru testing model by repurposing health centers’ parking lots to provide COVID-19 test services for community members.
- Managed central data registry of 30k+ patients’ COVID-19 test results; trained and supervised community health workers, clinic coordinators, and interns with appropriate delivery of test results, patient education, and data management.
Population Health Program Manager (June 2019 – Nov. 2021):
- Managed all grant-funded and research-based projects; supervised teams and developed database to monitor, track, and evaluate projects; co-chaired organization’s population health committee.
- Led interdepartmental teams and incorporated various quality improvement tools – PDSA, SWOT, flowcharts etc. to improve projects communication and collaboration.
Community Outreach and Enrollment Specialist (Jan. 2016 – June 2018):
- Conducted community outreach and formed partnership with community-based organizations, public libraries, faith institutions, school systems to educate community members about affordable health plan options offered through Obamacare/Affordable Care Act (ACA).
