I have worked with the elderly supporting their wellness journeys and social determinants of health. I have worked in the capacity of a Psychiatric Social Worker, I have worked with the most urgent and compromised patients throughout their acute psychiatric stages of wellness. In this process I supported the patients success with connecting them and or reconnecting them with family and or community supports. Previously, I worked within one of the largest New York
State Health Systems, providing support and linkage to care to HIV positive youth/ young adults as well as HIV negative youth/ young adults who may be at risk of HIV infection. HIV negative young adults were engaging in conversations about risk reduction, prevention, and adherence/
disclosure counseling, and education regarding STI’s and prevention/protection.
I am skilled at conducting comprehensive psychosocial assessments, at providing motivational counseling, problem solving and task focused counseling and link the patients to appropriate aftercare programs.
I have an understanding of the impact of someone’s environment on their health and well- being and the need to address these social determinants in order to optimize a persons health through comprehensive assessments, engagement, motivational counseling, and community referrals.
