Newly graduate in occupational therapy, looking for a role in occupational therapy willing to relocate.
Managed a caseload of 3–4 patients, conducted Oxford Cognitive Screen and Addenbrooke’s Cognitive Examination assessments to identify cognitive or aphasic difficulties, and completed kitchen, shower, and grooming assessments. I contributed to MDT meetings, led discharge planning, and worked jointly with physiotherapists and speech therapists to coordinate holistic interventions.
Led initial home assessments following triage calls with patients or next of kin, conducted physical and functional assessments, and implemented short-term reablement plans. I liaised with Community Equipment Services to arrange same-day provision (e.g., walking aids, toilet raisers) and supported referrals for larger home adaptations via local authorities.
Delivered person-centred reablement programmes, carried out environmental assessments, and contributed to falls prevention education within care homes, promoting safe and independent living aligned with NHS “No Stumbles” guidelines.
